Request for Taxpayer Identification Number & Certification

Depending on your business structure, we may be required to report your non-employee income to the Internal Revenue Service.  IRS Form W-9 is how you officially provide us with your correct Taxpayer Identification Number (TIN), mailing address, and the type of business structure you operate as.

 

Important: We may ask you for a new IRS Form W-9 annually, or sooner if you move.  If you do not complete this form timely, the IRS requires we start backup withholding at the current rate of 24%.  You can learn more about IRS Form W-9 on IRS.gov.
